SCHEDULE 2Information in relation to transplants outside the United Kingdom

schedule 2 paragraph 1 + 1.  The full name of the relevant clinician suppling the information to the Human Tissue Authority, their contact details, the position they hold and the place at which they hold it.

schedule 2 paragraph 2 + 2.  The following information about the recipient—

schedule 2 paragraph 2 a + (a)full name,

schedule 2 paragraph 2 b + (b)date of birth,

schedule 2 paragraph 2 c + (c)home address,

schedule 2 paragraph 2 d + (d)age at time of organ transplantation,

schedule 2 paragraph 2 e + (e)gender,

schedule 2 paragraph 2 f + (f)whether the recipient was treated at the relevant clinician’s centre before travelling outside the United Kingdom for organ transplantation,

schedule 2 paragraph 2 g + (g)status of the recipient on the United Kingdom organ transplant list when they travelled outside the United Kingdom for organ transplantation,

schedule 2 paragraph 2 h + (h)whether the recipient was referred for organ transplantation outside the United Kingdom and, if such a referral took place, the reason or reasons for this,

schedule 2 paragraph 2 i + (i)country or countries of legal citizenship or residency.

schedule 2 paragraph 3 + 3.  The following information about the donor—

schedule 2 paragraph 3 a + (a)full name,

schedule 2 paragraph 3 b + (b)whether the donor was living or deceased at the time of the organ donation,

schedule 2 paragraph 3 c + (c)date of birth,

schedule 2 paragraph 3 d + (d)age at the time of the organ donation,

schedule 2 paragraph 3 e + (e)gender,

schedule 2 paragraph 3 f + (f)country or countries of legal citizenship or residency.

schedule 2 paragraph 4 + 4.  The relationship between the donor and recipient, if one exists.

schedule 2 paragraph 5 + 5.  A description of the organ believed to have been transplanted.

schedule 2 paragraph 6 + 6.  The date of the organ transplantation.

schedule 2 paragraph 7 + 7.  The country, city and centre where the organ transplantation took place.

schedule 2 paragraph 8 + 8.  Contact details for the clinician who performed the organ transplantation or the centre where the organ transplantation took place.
